Community Survey

The Community Survey is now closed!

The Jackson Collaborative Network implements a Collaborative Community Assessment process every three years that includes a Community Survey to gather data directly from community members county-wide.  This county-wide survey is designed to provide data that helps us understand community conditions specific to a variety of social drivers of health, including things like safety, housing, transportation, and social supports.
